A patient-specific consent should be assigned from the protected THCB clinical record and completed through the secure e-signature workflow. Do not use a public/shared blank PDF as the signed patient record.
Safety: Educational cards do not replace individualized assessment. Sudden vision change or loss, severe neurologic symptoms, difficulty breathing, or another potentially life-threatening symptom requires immediate emergency evaluation rather than waiting for an app message.
